Add composite index on sessions(user_id, accessed_at) - #57592
Input data is properly validated, SELECT * is avoided, SQL injection is prevented (using placeholders for values in statements), JS inline code is prevented especially for url redirects, and untrusted data interpolated into shell scripts/commands is validated against shell metacharacters.
Context
Every authenticated request runs UserByID, which includes a MAX(accessed_at) subquery on the sessions table to compute the user's last activity time. The sessions table only had a single-column index on user_id, so MySQL could quickly find "all sessions for this user" but then had to scan every one of those rows to find the highest accessed_at value.
For a normal user with a few sessions this is invisible. But accounts that accumulate sessions over time (shared admin accounts, CI/test accounts, accounts that rarely log out) grow this cost linearly. At 250k sessions for one user, the subquery scanned ~125k rows and added ~113ms of database time to every single API request made by that user.
What this PR does
Replaces the single-column idx_sessions_user_id (user_id) index with a composite idx_sessions_user_id_accessed_at (user_id, accessed_at).
With both columns in the index, MySQL can answer MAX(accessed_at) WHERE user_id = ? by reading the last entry in the index for that user, without touching any table rows at all. MySQL reports this as "Select tables optimized away" in EXPLAIN.
As a side benefit, the amountActiveUsersSinceDB query (WHERE user_id = ? AND accessed_at >= ?, used for Fleet usage statistics) also becomes a covering index scan.
Impact analysis
There are 11 queries in the codebase that touch the sessions table:
2 queries improve: the MAX(accessed_at) subquery (every authenticated request) and the active-users statistics query (both now fully covered by the index).
9 queries are unaffected: queries that look up by id or key use other indexes; queries that filter by WHERE user_id = ? still work identically because user_id remains the leftmost column.
0 queries are negatively affected. No code uses FORCE INDEX on the old index name. No foreign keys reference the sessions table.
The only added cost is that MarkSessionAccessed (which updates accessed_at on every request) now also updates the composite index entry. This is one B-tree update on a single row, negligible compared to the 113ms saved on the read side.
Testing
QA'd all new/changed functionality manually
Reproduction (local, MySQL 8.0.46)
Started a local Fleet server with an isolated database.
Created an admin user, inserted 250k sessions for that user.
Profiled the userLastActivitySelect subquery before and after the index change:
Query time
EXPLAIN
Rows scanned
Before (single-column index)
113ms
DEPENDENT SUBQUERY, key: idx_sessions_user_id
124,555
After (composite index)
0.2ms
Select tables optimized away
0
Verified GET /api/latest/fleet/me returns the correct last_activity_at value with 250k sessions present.

Database migrations
Checked schema for all modified table for columns that will auto-update timestamps during migration.
Confirmed that updating the timestamps is acceptable, and will not cause unwanted side effects.
Ensured the migration can be retried if it was partially applied after a failure.
Migration details
The migration drops idx_sessions_user_id and creates idx_sessions_user_id_accessed_at (user_id, accessed_at). Both steps are guarded with indexExistsTx checks, so the migration is safe to retry if it fails partway through.
The sessions table is small (one row per active login session, not per host), so the ALTER TABLE completes in milliseconds on any deployment.

Performance
Session lookups using both the associated user and last access time can now be more efficient, which may improve responsiveness when retrieving session information from larger datasets. Session behavior and available functionality remain unchanged.
The userLastActivitySelect subquery (SELECT MAX(accessed_at) WHERE user_id=?)
runs on every authenticated request. With only the single-column user_id index,
MySQL scans all of a user's session rows. Accounts with many sessions (e.g. a
shared admin or CI account with 250k sessions) add ~113ms of DB time per
request. The composite index lets MySQL resolve the MAX via the index alone
("Select tables optimized away"), reducing that to <1ms.

🟡 Changes recommended
The migration drops the existing index before creating its replacement, so a failed or interrupted index creation can leave queries filtered by user_id scanning the sessions table.

Fixed. Swapped the order so the composite index is created first, then the old one is dropped. This way the sessions table always has a usable user_id index even if the migration is interrupted.
Adds a MySQL migration that creates the sessions index on (user_id, accessed_at) if absent, then drops the index on user_id if present. The down migration makes no schema changes. The schema snapshot records the new index, migration version, and updated auto-increment value.

Explanation
The description follows the repository template in substance. It links the related issue (#55579), includes a context section, a change summary, an impact analysis, testing notes with reproduction data, the required AI section, and database migration checks. The template's Testing items, Changes file item, and other checklist items are not all marked. The description also omits the 'Added/updated automated tests' item and the changes file, and does not mention a changes/ file for this user-visible-performance change. These gaps are non-critical, so the description is mostly complete.

Explanation
Issue #55579 requires that authentication cost does not grow with the number of sessions a user has. The reviewed head adds migration Up_20261104165437, which creates idx_sessions_user_id_accessed_at (user_id, accessed_at) and drops idx_sessions_user_id. The schema.sql change makes the same index change. With this index, the MAX(sessions.accessed_at) subquery in userLastActivitySelect can read the last index entry for the user. The PR reports a local MySQL 8.0.46 test with 250,000 sessions: 113 ms and 124,555 rows scanned before the change, and 0.2 ms with 0 rows scanned after it. The PR also reports that GET /api/latest/fleet/me returns the correct last_activity_at. The change since the previous review adds the migration file and the schema.sql metadata. It does not alter the query logic. The previous conclusion still holds. Issue #55579 does not require session cleanup, so the absence of cleanup code is not a gap. Evidence limit: the reported timings come from the PR author. This review did not rerun them.
Full details: Out of Scope Changes check
Explanation
The reviewed changes are the new migration file 20261104165437_AddSessionsCompositeIndex.go and the matching schema.sql update. The schema.sql update sets migration_status_tablesAUTO_INCREMENT to 694 and records version 20261104165437 as applied. This follows from adding one migration. Both are required for the index change to apply on new and upgraded installs. The migration checks index existence before each step, so retries are safe. No unrelated implementation appears in the reviewed changes. Session cleanup is not part of this PR and is not required by #55579.

Add the composite index before dropping idx_sessions_user_id.
Dropping first leaves sessions without a user_id index. Instances that keep serving can run userLastActivitySelect during that window. DROP INDEX commits implicitly, so a failed add leaves no index until a retry succeeds. Create the composite index first.
